Catheter insertion 2. Record date and time catheter was inserted.

Catheter insertion 3. Record the name of the clinician who inserted the catheter.

Catheter insertion 4. Select the indication for catheter insertion.

Catheter insertion 5. Record whether this is the first insertion or if this is a catheter change.

Catheter insertion 6. Select the drainage bag used.

Catheter insertion 7. Record: - volume required to dilate the balloon - the volume of urine drained when the catheter was inserted. Total volume drained volume drained into the kidney dish volume drained into the drainage system at time of insertion

Catheter insertion 8. Indicate whether a urine specimen was collected at the time of insertion.

Catheter insertion 9. Describe the colour of the urine drained at time of insertion

Catheter insertion 10. Indicate whether a CAUTI has already been detected
