Who We Are Who We Are and How We Provide Support 270 Social Workers/Case Managers, Support Staff and Management employed by Ramsey County and 15 contracted community agencies. Responsible for providing case management to nearly 7500 people, 1200 under the age of 21. Charged with implementing the tenants of the Federal Olmstead Decision, the Minnesota Olmstead Plan and the Jensen Settlement. 2
How We Support Olmstead: Community Integration for Everyone In 2009, the Civil Rights Division launched an aggressive effort to enforce the Supreme Court's decision in Olmstead v. L.C., a ruling that requires states to eliminate unnecessary segregation of persons with disabilities and to ensure that persons with disabilities receive services in the most integrated setting appropriate to their needs. Lois Curtis and Elaine Wilson- were confined in an institution for several years after their treatment was completed. On June 22, 1999, the United States Supreme County Held that Olmstead v LC that unjustified segregation of person with disabilities constitutes discrimination in violation of title II of the Americans with Disabilities Act. President Obama issued a proclamation launching the "Year of Community Living," and directed the Administration to redouble enforcement efforts. ADA.gov 3
How We Support Jensen Settlement- Class Action Lawsuit The Jensen Settlement Agreement is the result of a lawsuit filed against the DHS in 2009 alleging that residents of the former Minnesota Extended Treatment Options (METO) program were unlawfully and unconstitutionally secluded and restrained. Rather than a trial, on Dec. 5, 2011, the U.S. District Court for the District of Minnesota adopted a Settlement Agreement (PDF). The settlement is intended to bring significant improvements to the care and treatment of individuals with developmental and other disabilities in the state of Minnesota. Olmstead/Jensen are essentially about ensuring civil rights and mandating integration for people with disabilities. MN.gov/dhs 4

How We Support Minnesota Olmstead Plan Responsibilities For counties, providers and other service organization To create a disability services infrastructure that ensures Minnesotans with disabilities have access to opportunities for living, learning, working, and enjoying life in the environment of their choices. It is not simply a matter of creating an independent environment for people with disabilities, but rather to allow them the power to make decisions about the direction of their lives. This power is evidenced by people who are integrated into the community of their choice, having control of the services they receive and funding sources, and having access to transportation and equal employment, education and housing opportunities 6
How We Support Civil Rights and the Dignity of Risk Ramsey’s Efforts Transform our part of the service delivery system from one which protects people to one that supports people in making their own choices. Focus is on helping people and their families see the possibilities for their lives beyond what is typically expected for people with disabilities. Adjust our approach to focus on what support someone needs to live their life in the manner they choose. 7

How We Support What is Case Management? Case management include but not limited to the following: Coordinate services Develop a service plan based on people’s assessed needs and preferences Evaluate and monitor services identified in the service plan Help people access needed services Help people identify potential service providers Inform people or their legal representative of service options Provide information about resources to the participants and/or the guardian. dhs.state.mn.us- CBSM 22
