P.A.T.H. PARTNERS ADVOCATING FOR TEEN HEALTH Servicing Kent County Teenagers - ages 13 – 19 Reaching out to Public Schools - mobile units - clinic Coordinating with Organizations, Physicians, and many other Service Providers of Kent County Hosting Various Health Promoting Community Events
Our Mission and Philosophy to Community Mission of P.A.T.H Providing communities with competent, caring health services focused on enriching the mind, body and spirit of today’s youth Philosophy of P.A.T.H Our philosophy at Partnership Advocating for Teen Health is based on values and beliefs that a partnership between nurses and the community will lead to a better future for today’s youth. We strive to provide quality, comprehensive mobile service, with a focus on health promotion, illness prevention, and improved lifestyles of teens. We value and aim to provide quality, trust, and respect, to the community. Adolescents have unique healthcare needs, and we believe that providing them with access to resources and education they will be likely to engage in responsible decision-making. We believe that by providing quality healthcare services, community resources, and education enrichment we will mitigate the prevalence of chronic health issues and behaviors to help build a stronger community.
WHY WE ARE NEEDED 23% of Michigan Households with Children have Food Insecurities 13% of Michigan Children did Not Receive Immunizations in 2012 Pregnancy affected the lives of 35 out of 1000 Teenagers in Michigan Michigan has the 3rd HIGHEST Obesity rate in the U.S. Michigan Teen Drug use is Higher than the National Average Almost 28% of Michigan Children have Not Seen a Dentist in 1 year
WHY WE ARE NEEDED 20.9% of Michigan residents live in poverty Nearly 3 million Michigan residents do not receive proper healthcare Nearly 14% of adolescents are uninsured 48% of adolescents receiving a D or F in school do not have proper check ups

Adolescent Health Goal Improve the development, health, safety, and well-being of adolescents and young adults. Increased Risk For: Suicide Motor Vehicle Crashes Substance Abuse Smoking Teen and Unplanned Pregnancies Evidence Indicates Need for the Creation of Successful Youth Development Intervention Programs
Organizational Goals The goal of our program is to provide education, support, and resources needed to reduce the occurrence of illness, injury, and obesity in teens and young adults through the development of program that increase the awareness of healthy behaviors; including healthy food and exercise to adolescents in Kent County.
Organizational Objectives Raise awareness of healthcare needs Develop partnership with area professionals as well as the community Develop healthy living program with meal plans, exercise, and health living skills
Organizational Objectives Increase the percentage of teens with dental and healthcare Decrease the rate of suicide amongst adolescents Reduce rates of unintentional injuries
Organizational Personnel and Position Descriptions Chief Executive Officer (CEO) – Manage PATH organization Director of Health Services – Report to CEO as well as work with other organizations Nurse Managers – Reporting to Director of Health Services, managing budget, and scheduling Nurse Practitioners – Accountable for treatment, performing physicals, and prescribing medications Staff Nurses – Work directly with patients and the community

Future of P.A.T.H Personal CEO and Director of Health Services Will also hire Youth Advisor and Exercise Specialists Increase capacity to do in-house diagnostics, such as pregnancy and blood tests One fixed office and two mobile units Assess health care needs of teenagers and will make changes to meet those needs
PATH - Partnership Advocating for Teen Health Registered Nurse Community based organization seeking highly-motivated, qualified RN to join our growing team. Must embrace the organizations philosophy to creating a better future for our youth by providing quality comprehensive services, focused on prevention of chronic disease and improving the lifestyle of youth within the community. This individual should also share the values of trust, quality care, respect, availability, and adequate resource allocation and strive to promote healthy behaviors through educational enrichment and provision of community resources aimed at building the foundation for a stronger community. Job Description and Essential functions: Provides professional nursing services to youth, families, and the community; assesses needs, initiates, implements, and evaluates individualized plans of care including referrals and collaboration with other disciplines; provides high quality, comprehensive services focused on preventative care; delivers safe, efficient, non-discriminatory services; delegates and supervises care of others; utilizes available internal and external factors to properly allocate resources; and maintains professional accountability of knowledge and safety.
