ACCEPTANCE CHECKLIST REQUIREMENTS Upload to myRecordTracker by July 30th Completed Health Form including Physical Examination (Section I) Immunization Records for Varicella, MMR, Hepatitis B, TDAP Hepatitis B Surface Antibody Quantitative Titer Results First Step of TB Testing (Second Step should wait until August) Flu Vaccination – Do not obtain before September (Due October 1st) Covid-19 Vaccination – Reply YES if or NO and upload vaccination card, if applicable

Instructions will be given at time of testing – 24 to 48 hours notice given Cost is currently 34 – keep money aside! Make sure you read and uploaded the Medical Marijuana Policy (Section M)
*** IMPORTANT – PLEASE NOTE *** Failure to comply with the established deadlines for document submission will result in cancellation of your acceptance! Acceptance is conditional upon approval of Background Clearances. Failure to comply with establish deadlines during the program for subsequent requirements such as Hepatitis B repeat doses, flu vaccinations, etc. will result in removal from the program. It is your responsibility to keep a copy of all documentation for your records. You will need them during the program as well as for future employment.
PREPARING FOR CLASS Uniforms Solid Caribbean Blue Scrub Top and Scrub Pants Need by mid-September Must wear on Lab Days, Observations, and Externship Can wear lab jacket (Caribbean Blue or White) if needed Can wear solid color (white, black, or Caribbean Blue) long-sleeved t-shirt under scrub top Closed-toe, solid white shoes or sneakers – no clogs or open backs Books Purchase at NCC Bookstores on Main or Monroe Campuses, or online at https://www.bkstr.com/northamptonccstore/home College ID Obtain an NCC Student ID at Main Campus, College Center, Room 200 or at Monroe Campus, Student Services. Bring a valid photo ID and a copy of your current course schedule.
